We would like to introduce you to our Carer Champions, Carol. She is here to coordinate the identification and support of carers within St Peters Hill Surgery, acting as a voice for carers and they are your key point of contact for any carer related information that you might need. Carers FIRST is a charity which supports people who look after a relative or friend who could not manage without their help. They work across areas in London, the South East and East of England.
They also offer support for young adult carers (aged 16-25) who help look after someone in their family, or a friend who is ill, disabled, or misuses drugs or alcohol.

Lincolnshire County Council Young Carers Service
Young carers can be a member of any family, from any background, and in any situation. The Lincolnshire Young Carers service helps and supports children and young people up to the age of 18 who are helping to care for someone at home.

The Lincolnshire County Council also run a Dementia Support Service, provided by the Lincolnshire Partnership NHS Foundation Trust (LPFT). The service gives you access to a support worker who will provide guidance and advice throughout all stages of your dementia journey.
